In order to begin Challenging your Mountain, there are a few forms that need to be completed. These are to ensure that all parties involved in the week are aware of the terms and conditions associated with The GYM's Mountain Challenge.

Step One

Physical Activity Readiness Questionnaire (PAR-Q) Form:
Download the PAR-Q Form. This form needs to be filled out to ensure you are healthy and fit enough to join our program.

Step Two

THE GYM Policy Form:
Download the Policy Form. This form needs to be filled out to show you understand and will abide by the polices set forth by THE GYM.

Step Three

Waiver, Release, and Assumption of Risk Form:
Download the Waiver Release Form. This form needs to be filled out to show you understand and accept the risks of the program that you will be participating in.

Step Four

The Gym's Mountain Challenge Registration Form:
Download the Registration Form. If you are physically fit enough to participate, accept the policies set forth by THE GYM, and accept the risks involved, you are ready to register!

After you have downloaded, read over, and filled out all the forms, you are ready to send them to the Gym. You can send us your forms either by:
